Our partners include healthcare providers, health plans, residential communities, community-based organizations, Area Agencies on Aging, and all others dedicated to improving the health of the communities we serve. Healthy Living for ME® (HL4ME®) contracts and supports partners with service implementation, delivery, quality, and other needs to reach more Mainers. Our programs provide a comprehensive and effective approach to improving the health and well-being of individuals within the community.
If you're interested in bringing additional services to your community, by volunteering, referring participants, hosting a program or becoming a network partner to provide services directly, we welcome you to join us.
