Healthy Living for ME is improving health and wellness in communities across Maine
Healthy Living for ME® (HL4ME®) is the Network Lead Entity (NLE) of Maine’s Community Integrated Health Network (CIHN) made up of local organizations, health systems, and volunteers who work together to empower individuals to take charge of their health. Through tailored services, we provide free and low-cost options that are personalized to focus on the whole individual rather than a single condition to improve overall quality of life.
HL4ME brings community-based organizations together under a single contracting structure to provide an array of social need and social care interventions to address gaps in the Social Determinants of Health (SDoH) for Maine residents of any age.
Our mission is to coordinate and align community resources to improve the health and wellness of the people of Maine. Our vision is to provide Maine people with the skills and resources to take control to build healthier lives.
