AUGUSTA — Healthy Living for ME is one of 20 grantees for USAging’s Center of Excellence to Align Health and Social Care.

With the funding received through the COE, the agency will be able to further develop HL4ME’s capabilities, capacities, and support infrastructure to better coordinate health and social care service deliveries for people in Maine. This will include coordinating and collaborating with current network partners and onboarding additional network partners of the Community Care Hub, as well as contracting with healthcare and insurance systems to provide social care coordination.

The grant amount cannot yet be shared publicly.

The intent of these awards is to provide infrastructure funding to the selected CCHs like HL4ME in support of their contracting efforts with health care organizations for coordinated access to and delivery of social care programs and services (including support for hospital-to-home care transition programs) that assess and address health-related social needs and improve health equity and inclusion for individuals, families, and/or caregivers.

These efforts will also enhance No Wrong Door access systems and coordinate with local and/or state public health departments.
