Healthy Living for ME
Maine's Community Care Hub
Our mission
Strengthen and sustain integration between health care and community-based organizations to improve whole-person health for all Mainers.
A better model for health
Maine's health care system and community-based organizations (CBOs) operate with disparate data systems, workflows, and inequitable payment structures. This makes it very difficult to effectively address Mainers' intersecting medical and health-related social needs. As a community care hub (CCH), Healthy Living for ME serves as a bridge between health care and CBOs by centralizing administrative functions and operational infrastructure. This includes contract oversight, shared payment, management of a secure closed-loop referral system, service delivery quality assurance, data collection, and reporting. The result? A truly integrated and equitably compensated system that wholistically improves health outcomes. See a visual and read success stories of the CCH model.
Our work
Healthy Living for ME currently operates as a division of Spectrum Generations (the Central Maine Area Agency on Aging). Our secure data system, Cumulus, supports all aspects of our CCH infrastructure.
